ارزیابی کلینیکی و رادیوگرافیک تغییرات عرضی ریج آلوئول با استفاده از demineralized freeze-dried cancellous block allograft

چکیده

مقدمه و هدف: تحلیل استخوان فک پس از کشیدن دندان ها یک مشکل شایع بین بیماران است. در این شرایط قرار دادن ایمپلنت در استخوان و رسیدن به بقا و زیبایی مناسب با مشکل مواجه می شود.یکی از روش های درمانی جهت بازسازی استخوان از دست رفته استفاده از آلوگرفتهاست. اگرچه با این روش از مشکلات محل دهنده اجتناب می شود اما استفاده از این مواد در بازسازی های وسیع ریج نتایج متناقضی را نشان داده است. مطالعه حاضر با هدف بررسی نتایج کلینیکی و رادیوگرافیک استفاده از demineralized freeze-dried cancellous block allograft در افزایش عرض ریج بی دندان صورت گرفت. روش کار و مواد: ? بیمار (2 مرد و 5 زن با متوسط سن22/12± 28/38 سال) وارد مطالعه شدند. بازسازی طرفی استخوان با استفاده از بلاک های آلوگرفت دمینرالیزه اسفنجی همراه با پیچ های تثبیت کننده و غشا کلاژنی قابل جذب صورت گرفت. اندازه گیری عرض استخوان حین جراحی و 6 ماه پس از آن در زمان قرار دادن ایمپلنت به کمک استنت آکریلی و 3 میلی متر پایین تر از کرست باکالی دندان مجاور صورت گرفت. همچنین تصویر cbct همراه با استنت آکریلی دارای یک شاخص رادیواپک در ناحیه بی دندانی در ابتدای مطالعه و 6 ماه پس از جراحی از بیماران تهیه و اندازه گیری عرض استخوانی جهت ارزیابی مقدار استخوان بدست آمده انجام گرفت. یافته ها: میانگین افزایش عرض کلینیکی و رادیوگرافیک به ترتیب 90/1±70/1 و 52/1± 28/1 میلی متر محاسبه گردید که از نظر آماری این اختلافات معنی دار نبود. (به ترتیب 056/0p = و067/0= p) نتیجه گیری: نتایج این مطالعه نشان داد استفاده از بلاک های اسفنجی دمینرالیزه همراه با غشا قابل جذب در بازسازی نواقص عرضی ریج نتایج قابل پیش بینی ندارد و لذا انجام مطالعات وسیع تر و با تعداد نمونه های بیشتر جهت یافتن فاکتورهای مداخله گر پیشنهاد می شود
